1. Please provide your insurance information at least 24 hours before your appointment. If you are unable, we will try to verify your insurance benefits when you come in to the office, however if we are unable to verify your insurance, you may need to pay out of pocket and self-submit your claim.

2. Retinal Photos are required by our office and are not covered by most insurances.

3. We utilize telehealth technology for many of our exams. You will receive your glasses and contact lens prescriptions, and the doctor will evaluate your full eye health in your comprehensive eye exam.

4. If you are more than 10 minutes late for your check-in, we may have to reschedule.

5. Out-of-Network Patient Acknowledgement: We are out of network with several insurance providers. Please call our office to verify if we take your insurance before your appointment. If you are out-of-network and book an appointment, you are agreeing to paying the full price of the exam and understand that you will be reimbursed by your insurance based on your out of network benefits. Please reach out to your insurance for specific reimbursement amounts.
